Complaint submission guidelines

These rules apply to every complaint submitted for possible publication.

Submit your complaint

Who may submit

Adults may submit their own firsthand experiences as agents, brokers, employers, HR representatives, employees, policyholders, customers, claimants, vendors, or other people with a direct relationship to the events described. Do not submit a rumor, a story copied from social media, or an accusation on behalf of another person unless you are legally authorized and clearly identify the source.

What the submission may concern

The form may be used for an experience concerning Colonial Life & Accident Insurance Company, The Paul Revere Life Insurance Company, Colonial Voluntary Benefits, Kathy Pratt, Erika Wasiak, Raul Mota, or another company representative. Colonial Voluntary Benefits is named in the public lawsuit; the verified complaint alleges that it is a marketing or trade name through which Colonial and/or Paul Revere products are marketed and sold. Naming a company or person does not establish liability.

Truth, firsthand knowledge, and attribution

State what happened to you, what you personally saw or heard, the approximate dates, who was involved, and what records support the account. Clearly distinguish personal knowledge from inference, opinion, information in a document, and information another person provided. Do not knowingly submit false information or omit a material fact in a way that makes the account misleading.

Material that must not be submitted

Do not include Social Security numbers, payment-card numbers, bank information, policy or account numbers, medical information, private addresses, personal telephone numbers, personal email addresses, employee or policyholder records, information identifying minors, intimate material, credentials, passwords, threats, slurs, doxxing, confidential discovery, privileged communications, settlement communications, private recordings, trade secrets, or anything subject to a sealing, protective, arbitration, nondisclosure, or confidentiality order.

No harassment or coordinated pressure

A submission may describe the submitter's experience and opinion. It may not direct readers to contact, threaten, harass, review-bomb, dox, target, or interfere with any company, defendant, employee, employer, policyholder, witness, lawyer, judge, regulator, customer, or other person. It may not encourage interference with litigation, testimony, employment, customers, contracts, or settlement discussions.

Email verification and moderation

The submitter must confirm the email address. A verified email does not verify the truth of the complaint. Every submission remains private unless an administrator manually reviews and approves an edited public version. Email links open a protected review screen; they do not publish content merely by being opened.

Editing, rejection, correction, and removal

The site may edit or redact a submission for clarity, privacy, length, attribution, tone, search presentation, legal review, or consistency without intentionally changing its substance. The site may request support, decline publication without explanation, remove a complaint, add context, publish a correction, or invite a response. Publication is never guaranteed and does not mean the site independently verified or endorsed every statement.

Responsibility of the submitter

The submitter remains responsible for the submitted content and represents that the required attestations are true. This website is not the submitter's lawyer, does not create an attorney-client relationship, and does not make a filing with any court, regulator, insurer, company, or defendant.

Responses from named parties

A person or organization named in a published complaint may identify the exact statement disputed and provide reliable supporting material through the contact page. A concise response may be considered. A request does not guarantee removal, alteration, or publication of a response.
